Consent to receiving information about goods and services

The consent is drawn up as a separate document (Article 9(1) of Federal Law No. 152-FZ of 27 July 2006 “On personal data”) and is requested only when I open the partner catalogue myself and want to receive a discount code. The consent is not a condition of using the Platform or of community membership; declining has no effect on anything.

1. Data subject

Full name, account email address, member number: filled in from the profile.

2. Operator

Intellectual Medical Technologies LLC (OOO «IMT»), OGRN 1269600002770, INN 6679188579, address: 620024, Sverdlovsk Region, Yekaterinburg, Bisertskaya St., 16, bldg 1, email privacy@healthrating.net.

3. Purpose of processing

One purpose: promoting the goods and services of the Platform’s partners and the Operator’s own services, including seasonal community events, by sending me informational and advertising messages (Article 15(1) of the Personal Data Law; Article 18(1) of Federal Law No. 38-FZ of 13 March 2006 “On advertising”; Article 44.1 of Federal Law No. 126-FZ of 7 July 2003 “On communications”).

4. Personal data

Full name, email address, member number, account country, the fact of community membership, device identifier for push notifications.

Health data, level, test results and marked conditions are not used for this purpose: the partner catalogue is the same for everyone, no personal selections are built.

5. Actions, methods, channels

Collection, recording, storage, use, blocking, deletion, destruction; automated processing. Channels: messages in the account, HemoTrack push notifications, emails. Frequency: no more than two messages a month, not counting notifications about a code issued to me. Every message contains a way to opt out.

6. Persons processing data on the Operator’s instructions

Selectel JSC (INN 7810962785, 196006, St Petersburg, Tsvetochnaya St., 21, lit. A) and an email delivery service operating in the Russian Federation; both are named in the Personal data processing policy. The Platform’s partners receive none of my data: they receive only confirmation that a code is valid.

7. Term and withdrawal

The consent is valid from signing until withdrawn. It can be withdrawn in the account settings, via the link in any email or by a request to privacy@healthrating.net; messages stop within 3 working days. Withdrawal does not cancel codes already issued to me and does not affect my community membership. Withdrawal does not affect the lawfulness of processing carried out before it.

8. Signing

The consent is signed with a simple electronic signature: by pressing “Receive offers” in an account whose access was confirmed by a one-time code sent to my email address, under Section 5 of the Public Offer. There are no pre-ticked boxes. The Operator stores the date and time of signing, the edition of the text and the account identifier.
