How it works

In short
You join the community and use the HemoTrack app. The app knows your age and sex, tells you which check-ups you’re due and when it’s time for a blood test. Done? Tick it off. That’s how your care level grows.
What is the care level
There are seven levels, each with its own wreath. Your level shows how well you keep up with what’s due for you over the last year. Everyone has their own list: shorter for a younger person, longer for someone older. That’s why anyone can reach a high level.
Level 1 · Steel — Watching
Level 2 · Copper — Taking first steps
Level 3 · Bronze — Following the trend
Level 4 · Silver — Keeping the rhythm
Level 5 · Gold — In control
Level 6 · Rose gold — All on track
Level 7 · Platinum — Caring for family and sharing
What doesn’t affect your level
Illness and family history. Your test results, good or bad. How much money you’ve spent. Inviting friends.
When your level might go down
What counts is what you’ve done over the last year. If something important is about to drop out of that year, the app will warn you two weeks ahead and suggest what to do. There are no penalties.
More on how it’s calculated
Care points
Each thing you do earns care points. Some count for more than others. A check-up from the state programme or a retest counts for more than a daily blood pressure reading. So a missed check-up shows up more than a missed reading.
The app shows points in small print. What matters most on the screen is your next step and what will help your level.
The last year
Everything you’ve done in the last 365 days counts. Each day, new things come into that period and things from a year ago drop out. There’s no reset day and no penalties.
If something you’ve done is about to drop out and your level would go down because of it, we’ll warn you 14 days ahead. We’ll tell you what’s dropping out and what will fix it. A hard time in life isn’t punished. Your level simply adjusts to what you’re doing now.
What your level is made of
Everything you do falls into four groups.
- Check-ups. What’s due for your age and sex under the state programme. Also follow-ups if you’ve noted a condition, and comparing your results with the previous ones every three months.
- Response. Retesting or seeing a doctor if a result is outside the lab’s range.
- Self-monitoring. Home readings: blood pressure, weight and others. The readings diary is always free.
- Family. Family members who have their own results or readings in the app.
If you don’t have one of the groups, it doesn’t pull your level down. For example, if none of your results were outside the lab’s range. Its weight is shared among the other groups.
It pays to note your condition: the check-ups you need get added to your list, and they raise your level. Hiding a diagnosis to get fewer things to do makes no sense.
How to check the calculation and disagree with it
Your web account has a screen that explains what your level is made of and what will change it. If you disagree with the calculation, you can file an objection there. We’ll review it within 30 days.
