This consent is requested when you join the community. Without it the HemoTrack app works in full: only the level, member number and privileges are unavailable.

I, the user of an account confirmed by a code sent to my email, give consent to OOO «IMT» (Intellectual Medical Technologies LLC, OGRN 1269600002770, INN 6679188579, address: 620024, Sverdlovsk Region, Yekaterinburg, Bisertskaya St., 16, bldg 1) to process my data for one purpose: calculating my level and care points and taking part in the HealthRating community.

What data

  • Facts of completed calendar tasks: date, task source (the state check-up programme, the follow-up schedule for a condition I marked, community rules, a repeat test, a week of self-monitoring), category.
  • Age and sex for the age-based check-up calendar; conditions I marked for the follow-up calendar.
  • Details of the report that confirms a task: laboratory, order number, file hash. They are needed so that one report counts once.
  • Regularity of home readings, but not their values.
  • The composition of my family on the platform for the family level.

Reading values and diagnoses do not affect the level. The level depends only on what I do.

What is done with it

Collection, recording, systematisation, accumulation, storage, updating, retrieval, use, anonymisation, blocking, deletion and destruction, using automated means.

What I know about the level

  • The level has no legal consequences: it does not affect the price or availability of services, is not passed to third parties and is not used for decisions about me.
  • Only I see my level. My family sees the overall family level, not my personal one.
  • I can see how the level is calculated at any time and file an objection in the account. The operator replies within 30 days.
  • Anonymised community statistics are published only for groups of at least 50 people.

Term and withdrawal

The consent is valid while I am a member of the community. It can be withdrawn in the account settings: this means leaving the community. The level, points and participation history are erased; test results and readings stay in my account and the app keeps working. Withdrawal does not affect the lawfulness of processing before it.

The consent is given electronically by pressing “Join” after reading the manifesto and the community rules, in an account whose login was confirmed by a code sent to my email. The date and time of consent, the version of this text and the account identifier are stored by the operator.
